# Phiếu khảo sát

#### Bước 1: Tạo đề khảo sát

* Truy cập vào trang admin mục Đề thi > Ngân hàng đề thi> Tạo đề thi
* Điền các thông tin : Tiêu đề, mô tả ,thời gian làm , câu hỏi....
* Lưu ý : Các câu hỏi trong đề phải là các câu trắc nghiệm(đúng sai, một lựa chọn, nhiều lựa chọn)

#### Bước 2: Thiết lập đề khảo sát vào khóa học

* Vào chi tiết từng khóa học > Click tab Thông tin , chọn bài khảo sát tương ứng với khóa học

* Chọn thời điểm làm bài khảo sát : điền % hoàn thành khóa học (học viên sẽ phải làm bài khảo sát khi đạt đủ % trên)

#### Xem báo cáo khảo sát

Sau khi học viên đã làm bài khảo sát, truy cập mục Báo cáo  > Khảo sát 

Click icon mắt để xem chi tiết từng khảo sát tương ứng của từng khóa học
